Please find attached to this email an FOI request regarding the welfare of the child requirement in respect of fertility treatment and other treatments for the purpose of assisting women to carry children.

Response

NHS Sheffield CCG is unable to answer. You will need to contact our provider Sheffield Teaching Hospital NHS Foundation Trust (sth.foi@nhs.net). The Assisted Conception Unit make the decision on eligibility for fertility treatment, the CCG makes the decision on whether or not exceptional funding is available for fertility treatment.
