Please supply me with the number of NHS Patients sent to Wood Hill Grange Care Home, Grimesthorpe Road, between 1^st October and 31^st December 2019. I would like the numbers on a daily basis please.

Response

The CCG can advise that less than 5 patients were admitted to Wood Hill Grange for the specified period. Please note that due to small numbers we cannot provide exact figures as this may lead to the identification of individuals if used in conjunction with information already available in the public domain.

Please be advised that Horizon who own the home also have other provision which may be accessed in Sheffield either from a commissioning perspective or via self-funders, individuals and families directly.
