This is a freedom of information request.   I seek information on the following:  

1. In your CCG, following dentoavleoar trauma, is there a minimum number of lost teeth required to satisfy receiving funding for replacement of lost teeth with dental implants? 
 
2. If so, what is that number?
 
3. Does each individual patient have to undergo an Individual Funding Request?
